Admissions Process

The admissions process consists of completing the paperwork listed below, a personal interview with the designated school principal, and possible achievement testing.  The following must be in the student’s file for the application to be reviewed by the designated principal.

  1. Admission Application & Fee (non-refundable)
  2. Medical Release Form – completed in full
  3. Photocopy of the student’s Birth certificate – legal certificate from the Bureau of Vital Statistics
  4. Copy of Social Security Card
  5. Florida Health Forms 680 (immunization certificate) and 3040 (physical exam) – may be obtained from the school the student is now attending or from your pediatrician office. * Students entering kindergarten will need a permanent immunization form on file with the admissions office one-month before the beginning of school. REMINDER: ALL kindergarten, 7th, 8th, & 9th graders, along with new, out of state students, MUST have their Hepatitis B series shots of 3 injections.
    • OUT OF STATE STUDENTS: All students entering the Florida school system for the FIRST time must complete HRS Form 680 and HRS Form 3040 within one month of the enrollment. (HRS Forms 680 & 3040 are available at any private physician’s office, walk-in-clinic or health department.)
    • TRANSFER/IN STATE: If your child has had a physical exam during the year previous to application at NCS, and if a completed physician’s exam for that physical can be provided, the HRS Form 680 will be the only form needed. All students involved in sports must have a current physical.
  6. School Recommendation Form – filled out by a school administrator or teacher from your last school (grades 1-12)
  7. Copy of most current report card
  8. Copy of student’s latest achievement test scores showing stanines (grades 1-12)
  9. Pastor/Youth Worker Recommendation Form (grades 6-12)
