INSTRUCTIONS:
1. Read document completely. If you do not remember what is contained in the Acceptable Use Policy of your school's Parent-Student Handbook, you may download it here: (High School - Middle School - Elementary). Complete the reverse of the document. Parent and student signatures are required. Student must register the name, wireless MAC address and serial number of the device with NCS.
2. Devices using a Windows operating system must have an active antivirus system installed and operating system patches, service packs and updates must be current. Make sure these items are up-to-date BEFORE bringing the device to school. Antivirus protection is STRONGLY encouraged for other operating systems (Mac, Linux, iOS, Android, etc.), including cellular phones.
3. Bring the device and the completed, signed form to the NCS IT Department. An appointment is recommended, as we are often working away from our office. Ask your teacher to contact us in advance, or email tech.support@nck12.com and we'll be happy to set a time to help you.
INSTRUCTIONS:
1. Complete and return the signed form to your teacher. A form is needed for each teacher, as permission to use a cell phone e-reader is granted on a per-teacher basis.
2. If the student needs to connect to the NCS wireless network, a NCS Network Device Registration Form must also be completed and processed through the NCS IT Department.
